Before any request for funding can be considered, this request for funding form must first be completed and submitted to the Kiwanis Youth Services Committee.  When you are finished completing the form, click the "Click Here to Send Request" button at the bottom of the screen and the information will be automatically sent to me.  If you want to keep of record of what you send, print the screen before you submit the report.
